Pharmacology

Thiamine is an essential coenzyme for carbohydrate metabolism. Thiamine deficiency develops when the dietary intake is inadequate; severe deficiency leads to the development of a syndrome known as beriberi. Chronic 'dry' beriberi is characterized by peripheral neuritis, bradycardia muscle weakness and para1ysis. Acute'wet' beriberi is characterized by cardiac failure and oedema. Demyelination of the central nervous system may develop in severe cases of thiamine deficiency.

Riboflavin is essential for the utilisation of energy from food. The active flavine mononucleotide and flavine adenine dinucleotide are involved as coenzymes in oxidativelredustive metabolic reaction. Riboflavin deficiency develops when the dietary intake is inadequate. Deficiency leads to the development of a well-defined syndrome known as ariboflavinosis, characterized by cheilosis, angular stomatitis, glossitis and seborrhoic keratosis of the nose and anogenital region. There may also be ocular symptoms including itching and burning of the eyes, photophobisa and corneal vascularization.

Pyridoxine is involved principally in amino acid metabolism, but is also involved in carbohydrate and fat metabolism. It is also required for the formation of haemoglobin. Pyridoxine deficiency in adults leads to the
development of peripheral neuritis, deficiency in children also affects the CNS. It is used in the treatment sideroblastic enemies, in certain metabolic disorders.

What Nicotinamide and Nicotinic acid occurs naturally in the body are vitamin B substances, which are converted to nicotinamide adenine dinucleleotide(NAD)and nicotinamide adenine dinucleotide phosphate
(NADP). These coenzyzymes are involved in electron transfer reaction in the respiratory chain. icotinamide and Nicotinic acid are used in the treatment and prevention of nicotinic acid deficiency. Its deficiency develops when the dietary intake is inadequate. Deficiency leads to the development of the syndrome known as pellagra, characterized by skin lesion, especially on areas exposed to sunlight. with hyperpigmentation and hyperkeratiniztion. Other symptoms include diarrhoea, abdominal pain, glossitis, stomatitis, loss of appetite, headache, lethargy, and mental and neurologial disturbances.

Dexpanthenol is the alcoholic of D-Pantothenic acid and is considered to be a vitamin B substance. It has heen used in a variety of conditions including controlling gastrointestinal atony.

Contra-indications

It is contra-indicated in haemophiliacs and in patients with ileus due to mechanical obstruction.

Precautions/Warnings

Pemacomb use should be used only under medical supervision.

Interactions

Pemacomb, consisting of pyridoxine (B6), has the following interactions considered important:

Antibiotics, Tetracycline -- All B complex vitamins, including vitamin B6, interfere with the absorption and effectiveness of antibiotic tetracycline. You should take tetracyclien at different times from vitamin B6 and other B vitamins.

Antidepressant Medications -- Taking vitamin B6 supplements may improve the effectiveness of certain tricyclic antidepressants such as nortriptyline (Pamelor), especially in elderly people. Other tricyclic antidepressants include amitriptyline (Elavil), desipramine (Norpramin), and imipramine (Tofranil).

On the other hand, antidepressants called monoamine oxidase inhibitors (MAOIs) may reduce blood levels of vitamin B6. Examples of MAOIs include phenelzine (Nardil) and tranylcypromine (Parnate).

Chemotherapy drugs -- Vitamin B6 may reduce certain side effects of 5-fluorouracil and doxorubicin, medications used to treat cancer, without reducing the effectiveness of the chemotherapy. Talk to your doctor before taking any supplement if you are undergoing chemotherapy.

Erythropoietin (EPO) -- Erythropoietin therapy, used to treat severe anemia, may decrease vitamin B6 levels in red blood cells and may require B6 supplementation.

Levodopa (L-dopa) -- Vitamin B6 reduces the effectiveness of levodopa, a medication used to treat Parkinson's disease. However, your doctor may be able to determine a dose of B6 that can help reduce side effects of levodopa without interfering with the drug's action. Taking vitamin B6 along with levodopa should be done only under the strict guidance of a physician.

Phenytoin (Dilantin) -- Vitamin B6 reduces the effectiveness of phenytoin, a medication used to treat seizures.

Adverse Effects

Rarely hypersensitivity reactions or allergy and other side effects are associated with long term administration of large doses.
