Pharmacology

Eldervit-12 is a combination of vitamins endowed with a particular hepato-protective activity in an optimal ratio so as to obtain the maximum therapeutic synergy. It also acts at the level of bone marrow to stimulate erythropoiesis. Vitamin B12 has a complex biological activity and takes part in protein synthesis particulally in neuronal and hepatic tissues.

Folic acid, besides being a haematinic factor, has a hepatic protective action. Whereas Niacinamide is known as enzymatic activator of biochemical cell processes. Vitamin C which is contained in a separate ampoule, when mixed with vitamin B12,Folic acid and Niacinamide, has a unique activity in the metabolic cycle which is necessary to the cellular structure.

Depending on the level and type of anaemia, Eldervit-12 injection is indicated for the following conditions:

  • Mild Anaemia
  • Moderate Anaemia
  • Post blood profusion maintenance of haemoglobin
  • Chronic or Severe anaemia.

Pharmacokinetics:

Absorbtion: Eldervit-12 injection, when given intramuscularly, achieves a plasma peak level in an hour while an intravenous infusion over 2 hours achieves a plasma peak in less than 30 minutes. Detectable concentrations occur as long as 48 hours after injection.

Distribution: Eldervit-12 injection is well distributed in all body tissues. Both direct and indirect evidence suggest that over 66% are distributed in bone tissues while the rest 34% are found in non bony tissues of the body. High bone concentrations occur in active bone marrow while non bony tissues concentration increase relatively in active growth as in wounds, trauma, lactation etc. The concentrations of Eldervit-12 injection are higher in bony tissues (up to 75%). Higher concentrations also occur in soft injuries – open or closed wounds.

Indications

Eldervit-12 injection has been found an effective adjunct in the management of cases in medicine, general surgery, obstetrics and gynaecology, orthopaedics.

General Surgery: pre and post operative care.

Orthopaedics: Eldervit-12 injection is useful in bone injuries when it supports bone marrow activity in the production of haemoglobin as well as stimulation of nerves. In long standing cases of immunesupression, head injury, fractures etc.

Internal Medicine: Eldervit-12 is an important adjunct in the management of hepato-cellular dysfunction/liver disorders, infections, general debility, weakness, hospitalised and convalescing patients, typhoid fever, malaria, jaundice or after any illness. Peripheral neuropathies, alcoholic, diabetics, hyperhomocysterinemia & related risks, megaloblastic anaemia; in prevention of neural tube defects, stroke patients and chronically ill patients also benefit from Eldervit -12 injection.

Elderly: Eldervit-12 is indicated for routine use in the elderly due to its action on neuronal activity. It reduces tremors and improves strength.

Obtetrics: Hyperemisis gravidarum: Routinely in antenatal visit, and in all cases of haemoglobin less than 11 g/dl during pueperium and lactation.

Gynaecology: Menorrhagia (not requiring surgery), chronic pelvic pain, etc.

Special indications: Pregnancy induced hypertension. A study on the usefulness of Eldervit-12 injections is not yet conclusive.

All cases of reduced appetite

All cases where blood transfusion is contra-indicated e.g. Jehovah’s Witness.

Contra-indications

Eldervit -12 injection is relatively contra-indicated in cases of diagnosed cancers. It is also contra-indicated in patients sensitive to any of its components.

Precautions/Warnings

Do not use after the expiration date.

In case of overdose, liberal fluid intake will relieve anxiety and any associated symptoms within 3-6 hours.

All possible causes of anaemia e.g. malaria, bleeding sites, infections, etc should be checked and treated while on Eldervit-12 injections for its quick response.

Interactions

The most significant result of Eldervit-12 injection is achieved when taken with a course of iron supplement. No adverse drug interaction has been reported when Eldervit-12 infusion is given through Metronidazole or Ciprofloxacin infusions.

Adverse Effects

No significant adverse reactions have been reported throughout the world with the use of Eldervit-12 injections. Pain at the point of injection is transient and stops as soon as the needle is removed.

Dosage & Administration

Intramuscular (IM):

Routine Antenatal care: Mix contents of part 1 ampoule with content of part 2 and give once a week. Stop if haemoglobin is > 13 g/dl and recommence if haemoglobin (Hb) < 11 g/dl.

Mild anaemia without active blood loss: part 1 & 2 twice weekly. Moderate anaemia part 1 & 2 everyday till normal non pregnant haemoglobin or maximum of 13g/dl for pregnant mothers is achieved

Severe anaemia: due to acute blood loss and for which blood transfusion is contraindicated e.g Jehovah’s Witness, part 1 & 2 daily till haemoglobin improves.

Intravenous IV):

Dosage same as for intramuscular but should be mixed with 500 ml of infusion and given over 2 hours.

Direction for use:

  • Sterile pack syringe and needle
  • Couple needle
  • Remove needle shield
  • Syringe and needle ready for use
  • Break ampoule 1 & 2 and draw up both in one syringe.

Storage/Handling Recommendations

Store below 25oC. Do not freeze. Protect from light.
